Older adult falls are the leading cause of injury and death for older adults. This presentation highlights how a statewide organization coordinated the development of an innovative toolkit designed for and by EMS and community-based organizations that outlines opportunities for collaboration and steps to implement identified strategies (assessment/referral tools, evidence-based programs, community support resources, and more) to prevent falls. Attendees will learn how preventing falls can save time, money, and resources and the collaborative development process of the toolkit, key lessons learned, and how to integrate the toolkit to improve fall prevention efforts and community health outcomes.
