What does an EMS doc, paramedic, and blood bank director have in common? They are all essential ingredients to a successful while blood field program. Hemorrhagic shock is one of the most time critical prehospital patient presentations. Medical literature clearly demonstrated the morbidity and mortality benefits in early administration of blood products in appropriate prehospital patients. While challenges exist in implementation, EMS clinicians are uniquely positioned to bring whole blood to the patient. Most presentations on the topic come from large agencies partnered with a level 1 trauma center, but understanding the dynamics of building a program no matter the EMS agency size or community resources is essential. Using first-hand experience EMS medical directors, paramedics, and blood bank leaders from systems large and small, we will provide a crash course on the operational, quality, and educational considerations a department must undertake. The critical branch points, pitfalls, and avenues for success will be covered. Key elements of partnership development and team creation is paramount will be stressed. A key focus will be on reducing barriers for applicability of program development in most system. At the end, the participant will have a digital toolkit they can leave with to help build their program. This group of panelist spent 1 year building prehospital whole blood programs in both large and small EMS systems. They hold expertise in quality, education, operations, and program development that spans EMS medicine, administration, and blood bank operations. Their work has led to the development of the complete package of needs that most systems can utilize to implement a whole blood program into large, medium, and small EMS systems. Throughout their combined experience, critical barriers have been identified and overcome making them uniquely qualified to offer their experience and recommendations for further expansion of whole blood into the prehospital arena.
