Ventricular tachycardia (VT) storm is a life-threatening cardiac emergency characterized by recurrent episodes of sustained ventricular arrhythmias requiring repeated intervention. Successful management requires rapid recognition, coordinated multidisciplinary care, and a thorough understanding of antiarrhythmic therapy, device management, sedation strategies, and escalation to advanced therapies. Nurses and paramedics play a pivotal role in the early identification, stabilization, and ongoing management of these critically ill patients. This presentation reviews the management of a patient presenting with VT storm, from initial assessment through definitive treatment. Key clinical decision points include recognition of electrical storm, pharmacologic interventions, synchronized cardioversion and defibrillation, correction of reversible causes, and coordination with electrophysiology and critical care teams.
